Registration with a chosen doctor

In order to make a visit with one of the doctors working in the polish Family Health Care centre one needs to make a registration for the appointment. Below are presented the currently available methods of patients' registration.

At the time of the first visit in our health care centre each patient is obliged to fill in a short form which will enable us to start the patient's record.

Ways to register

Telephone registration: It is possible to make an appointment with a chosen doctor by phone registration. In order to make an appointment please call one of the below listed phone numbers in the working hours of our health centre and give us the following information: first and last name of the patient, first and last name of the doctor and the preffered day and hour of the appointment. Phone numbers: - (01) 87 88 471 - 085 102 4218 Personal registration: It is also possible to make an appointment with a chosen doctor by a personal registration in the place of residence of the polish Family Health Care centre.
